How to submit an EasyPay Insurance Claim
- Complete the EasyPay Insurance Claim Form (available at any EasyPay Everywhere branch)
- Certified copy of the deceased’s death certificate
- Certified copy of the Notice of Death form (DHA1663)
- Proof of Identity for the deceased and claimant
- Claimants banking details (certified by the bank)
